Apparently the apps that use VoIP use a specific API that is always running on the device. That is, for our iPhone to immediately receive notification of an incoming VoIP call, that API is constantly running in the background on our iPhone.

And this is where the problem would be. Specifically, the constant execution of this API does not mean that the notification of the call is received instantly, but that by doing so it would be possible to obtain information from the users' devices.
This is why it will restrict VoIP calls from running through that API in the background. This means that apps that integrate VoIP calls that want to continue on iOS will have to adapt to this new change.
In fact, from Facebook they are already in contact with Apple to see how they can adapt their apps with VoIP to these new changes so that calls from WhatsApp and all its apps that allow these calls can continue to offer them.
